A krónikus fájdalom (3 vagy több hónapig tartó fájdalom) (1)
rendkívül gyakori közegészségügyi probléma (2) (3), és a legnagyobb
gazdasági terhet jelenti bármely kórállapotban (4). A krónikus, nem
rákos fájdalommal (Chronic Non-Cancer Pain [CNCP]) járó
állapotokban a radikuláris fájdalom rendellenességek (különösen a
hát alsó részén jelentkező fájdalom) esetében különösen nagy
arányban kerülnek felírásra opioidok (5), és a magasabb dózisok
ebben a krónikus fájdalommal élő betegkohorszban rosszabb
funkcionális kimeneteket vetítenek előre (6). A krónikus, nem
karcinóma eredetű fájdalommal járó állapotokban az opioidhasználati
zavarok (opioid use disorders [OUD-k]) aránya egy metaelemzés (7)
szerint 5%-tól 20-35%-ig terjed (8), (9), (10), (11). Mindezek
ellenére a krónikus, nem rákos fájdalommal élőknek felírt, hosszan
tartó opioidkezelések (chronic opioid therapy [COT]) száma jelentős
mértékben nőtt 1990 és 2010 között, és ez a jelenlegi opioidjárvány
egyik alapvető oka (12). Célunk egy olyan intervencióra alkalmas
szer kifejlesztése, amely radikuláris CNCP szindrómákban szenvedő,
közepes-magas dózisú opioidkezelésben részesülő betegek esetében az
opioidhasználatot olyan biztonságosabb dózisokra csökkenti, amelyek
mindazonáltal biztosítják vagy javítják a fájdalom kezelését.
